What the questionnaire does
It uses the name, WhatsApp number, selected concerns and preferred day you enter to prepare a message in your browser. This questionnaire does not submit your answers to a website database. You can review the message before opening WhatsApp.
When information is shared
Opening the WhatsApp link passes the prepared message to WhatsApp, which is a third-party service. Tap Send in WhatsApp to share it with the Malaya Men’s Wellness team for consultation follow-up. WhatsApp’s own privacy terms apply. Share only what you are comfortable sharing, and avoid sending identity documents or detailed medical records through this questionnaire.
